Overview: This position serves a young adult through older adult population with a strong interest in working as a care manager for addressing depression, anxiety and behavioral aspects of chronic conditions. This also includes but is not limited to adult eating disorders and other addictive disorders.

Core Competencies:

-Health psychology, primary care, and complex care management is preferred.

-Familiarity with behavioral Health integration strategies for primary care and appropriate documentation and billing.

-Demonstrated ability to use electronic medical record systems.-Experience with brief counseling/psychotherapies, motivational interviewing or behavior change techniques.

-Strong background working in medical/healthcare settings and working on multi-disciplinary teams is strongly preferred.

Educational Requirements:

Master’s Degree in Social Work (MSW) from a Council on Social Work Education (CSWE) accredited School of Social Work

License and Certifications Requirements:

Licensed Clinical Social Worker (LCSW),

Certified Social Worker in Health Care (C-SWHC)
