GreenField Health is the first provider in Oregon to implement Samepage Health’s innovative team-based program which optimizes comprehensive health management.  Samepage brings together clinical experts from across disciplines to help patients meet their health care goals through a systemic approach of check-ins with a GreenField health professional and weekly progress reviews. It focuses across a patient’s complex needs, ensuring that nothing is left out of a complete health care plan.

For example, a patient with diabetes might also struggle with depression, making healthy food choices or hypertension. The integrated Samepage system will coordinate care between the patient’s primary care provider, a mental health doctor, internist, nutritionist, care manager and any other staff appropriate to the circumstances. By working together, they make sure that not only is the diabetes treated but also that the patient has the tools and support to cope with and overcome the other barriers to good health.

“Often a short, intensive intervention is all that is required to help patients back to a place where they can focus on their long-term health needs,” said Dr. Traci Rieckmann, GreenField’s Chief Operating Officer. Dr. Rieckmann worked with Dr. Paul Ciechanowski, Samepage Health’s chief medical officer, on the research and training behind this program.  “In our academic research and real-world implementations of this program, we have consistently shown that using a coordinated health care team approach is the best way to achieve optimal outcomes in the shortest amount of time when helping patients with concurrent medical and behavioral issues,” Dr. Ciechanowski said.

Ensuring that GreenField patients have the support and top-quality care they desire, the team has worked closely with Samepage over the past several months to integrate this program in current clinical services at both clinics.  

Researchers at the University of Washington, including Dr. Ciechanowski, developed and refined this approach over two decades. It is an evidence-based system that has been peer reviewed in medical journals including the Journal of the American Medical Association and the New England Journal of Medicine. 

“Patients will get an opportunity to receive extra care for concurrent issues like depression. They’ll have regular contact with care managers who will help them focus on lifestyle change, self-management of their chronic diseases, and longer-term care,” GreenField founder Dr. Chuck Kilo said. 

Greenfield has worked closely with Samepage for the past six months to integrate the program into services at both its East and West clinics so that all patients will have the support and top-quality care they desire.
